Using Your InteliChart Patient Portal

We made an investment in your patient experience because we want you to have your health information right at your fingertips.

Here are some of the things you can take advantage of when you have the Patient Portal:

  • Request your  medication refills online
  • Securely communicate between you and your provider
  • The ability for the parent/guardian or patient to view upcoming appointments

How To Register For Intelichart

Lets walk through how to register for Intelichart.

If you find yourself needing more detailed instructions, please use this link  or en espanol aqui.

  • Step 1 - Request PIN

    From the email you received, click SET UP MY ACCOUNT.

    Then choose how you want to receive the PIN. Text is recommended.

    Note: If you received emails for additional children who are patients of the practice, you need to do this from each email. Each patient needs to be assigned a unique PIN.

    Proceed when you’ve received the PIN(s).

  • Step 2 - Confirm

    Click START MY REGISTRATION from one browser tab and close the additional tabs. Enter your email address, create a password, and click CONTINUE. Only do this once. If you have multiple PINs, you’ll add them next.

    Click Yes because you have a PIN and click Child or Dependent (unless you are the patient and you’re registering yourself for the portal.)

  • Step 3 - Child/Dependent Info

    If you picked Myself in the previous step, proceed by entering your info.

    Otherwise, enter the child’s or dependent’s birthday and PIN. Refer back to the text you received with the PIN. If you received PINs for additional patients, click

    + Add another child or dependent and complete their information. Click CONTINUE when you’re done.

  • Step 4 - Parent/Guardian Info

    Complete all of the fields with your information, select a Security Question, and provide the Answer. Then, review the Terms & Conditions and confirm that you did so by selecting the checkbox. Click CREATE MY ACCOUNT when you’re done.

    You’re all set!

    I currently have a portal, do I need to register again?

    Patients who were registered for the legacy OP Patient Portal are not carried over into the New Patient Portal, powered by InteliChart. Re-registration is required.

    Is there a mobile app?

    Yes, a free Patient Portal app can be downloaded onto your mobile device. Simply search for InteliChart in the App Store, and download “Patient PORTAL by InteliChart“.

    The following QR codes can also be used to instantly access the app for download.
